What Youll Do
- Deliver direct nursing care under the direction of a Registered Nurse (RN) in accordance with the plan of care
- Administer medications and treatments as ordered by the physician
- Monitor and report changes in the patients condition and ensure timely communication with the RN and interdisciplinary team
- Provide education to patients and caregivers on self-care medications and symptom management
- Assist with daily living activities to promote patient comfort and independence
- Maintain accurate clinical documentation and timely reporting of patient care and progress
- Participate in interdisciplinary team meetings quality improvement initiatives and continuing education
- Ensure safe respectful and compassionate care at all times
Qualifications
- Licensed Practical/Vocational Nurse (LPN/LVN) in the state of employment
- Graduate of an accredited Practical or Vocational Nursing program
- Minimum 1 year of nursing experience (hospice home health or related preferred)
- Active Basic Life Support (BLS/CPR) certification
- Valid drivers license auto insurance and reliable transportation
- Strong communication and time management skills
- Comfortable working independently and in rural or home settings
- Demonstrates compassion empathy and a deep understanding of end-of-life care
