Licensed Practical Nurse Med 3 (Nights)

The Licensed Practical Nurse (LPN) provides direct patient care under the direction of a Registered Nurse (RN). This role supports patient assessments care coordination patient education and communication with the healthcare team to ensure quality compassionate care.

Key Responsibilities

• Assist with patient admissions discharges and ongoing care.

• Perform patient assessments and monitor changes in condition.

• Communicate patient updates and concerns to the RN and care team.

• Participate in bedside handoffs and patient safety initiatives.

• Provide patient and family education and support.

• Document care accurately in the electronic medical record.

• Collaborate with the interdisciplinary team to deliver high-quality care.

• Delegate and direct support staff activities as appropriate.

• Participate in unit meetings huddles and process improvement activities.

• Perform additional duties as assigned.

Qualifications

Required

• Graduate of an accredited Practical Nursing program.

• Current LPN license in the state of practice.

• Ability to meet all State Board of Nursing requirements.

Preferred

• BLS certification (required within 30 days of hire if not already obtained).

• ACLS PALS PEARS S.T.A.B.L.E. or other unit-specific certifications as applicable.

• LPN-IV certification (or ability to obtain within required timeframe if applicable).
