Licensed Practical Nurse

Job Description Summary

We are seeking a Licensed Practical Nurse (LPN) to join our Home Health Team in Lee County. This role provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative rehabilitative palliative or preventative nursing care. Is responsible and accountable for making decisions that are based on the individuals nursing experience and educational preparation.

How will you make an impact & Requirements

• Patient Care: Provide high-quality nursing care including administering medications wound care and vital sign monitoring.
• Clinical Assessments: Conduct comprehensive assessments to identify patient needs and develop individualized care plans.
• Medication Administration: Accurately administer medications and educate patients on their medication regimen.
• Treatment Implementation: Implement prescribed treatments such as IV therapy and catheter care.
• Patient Education: Provide patient and family education on disease management prevention and self-care.
• Collaboration: Work collaboratively with physicians therapists and other healthcare providers to ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient care assessments and interventions.
• Infection Control: Adhere to strict infection control guidelines to protect both patients and healthcare workers.
• Emotional Support: Offer emotional support and compassion to patients and their families during challenging times.

• Licensure: Current and valid LPN license in the state of Florida.
• Experience: Minimum of 6 months of clinical experience as an LPN in a hospital or skilled nursing facility.
• Skills: Excellent communication interpersonal and critical thinking skills.
• Professionalism: Strong work ethic reliability and a commitment to providing quality care.
• Transportation: Reliable transportation to visit patients in their homes.

$34.49

The anticipated base salary range represents the Companys good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience skills qualifications geographic location internal equity and business needs.
