Position Title: Licensed Practical Nurse

Guaranteed Hours: 48/wk.
Work Schedule: Nights 7:00 PM To 7:00 AM
Pay rate: $45.00/hr - $50/hr.Responsibilities for this position include but are not limited to:
Skills:
- Administer medications and treatments as prescribed by physicians and in accordance with facility policies.
- Monitor residents vital signs health conditions and changes in physical or mental status.
- Provide direct nursing care to residents with chronic illnesses age-related conditions and other healthcare needs.
- Perform wound care dressing changes and routine skin assessments.
- Document nursing assessments medication administration treatments and resident progress in the EMR.
- Collaborate with registered nurses physicians CNAs medication aides and other healthcare professionals to ensure quality resident care.
- Assist with developing implementing and updating individualized care plans.
- Monitor residents for falls infections pain and other potential complications reporting concerns promptly.
- Support activities of daily living (ADLs) including bathing dressing mobility and personal hygiene as needed.
- Follow infection prevention safety and emergency response protocols.
- Communicate with residents and families regarding care needs and changes in condition within the scope of practice.
- Supervise and coordinate with nursing assistants as appropriate to maintain resident safety and quality of care.
