ACLDs Job Coach provides on-site job coaching and advocacy for people we support in the intensive and follow-along phases of Supported Employment.

In this position you will:

• Provide on-site job coaching for designated caseload
• Complete and ensure accurate and timely documentation of SAP plans case notes monthlies DDPs and other required documentation.
• Conduct observational assessments on site in order to assist the person supported in learning the tasks of the job
• Travel train on the public transportation system and/or roadways
• Maintain case files for each person supported.
• Prepare all documentation for ACCES and OPWDD on a monthly basis
• Act as a liaison between the person supported family and employer
• Attend all staff meetings Life Plans conferences and annual training requirements
• Provide overall assistance as assigned

We require:

• High School Diploma or equivalent; Associates Degree in Psychology Sociology Education or related field preferred.
• Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred
• Complete (within first year of employment) twenty-four (24) hours of OPWDDs Innovations in Employment Supports training.
• Clean valid New York State drivers license
