IMMEDIATE HIRE Hybrid RN G Tube Coordinator

Schedule: MondayFriday 8:00 AM5:00 PM
Hours: 40 hours/week
Work Arrangement: 90% Remote / 10% Onsite

We are seeking an experienced Registered Nurse to support members through the Arizona Division of Developmental Disabilities (DDD). This field-based position will complete nursing assessments document medical necessity coordinate services and collaborate with members families providers and multidisciplinary teams.

• Complete member reports and nursing assessments according to DDD requirements.
• Conduct in-person assessments every 90 days and as needed.
• Document medical necessity for skilled services.
• Complete nursing referrals and vendor calls.
• Participate in multidisciplinary planning meetings.
• Collaborate with members and their support systems.
• Educate members and families regarding complex medical information.
• Complete documentation within required timeframes.
• Attend mandatory meetings and complete annual training.
• Assist with special projects as needed.

• Active Arizona RN license.
• 45 years of recent RN experience in home health hospice long-term care or case management.
• Recent experience caring for individuals with feeding tubes including gastrostomy (G-tubes).
• At least 2 years of experience working with individuals with disabilities.
• Valid Arizona Drivers License.
• Arizona Fingerprint Clearance Card required.
• Moderate to advanced experience with Microsoft Word Google Suite and Adobe.
• Strong communication documentation and collaboration skills.
• Behavioral health complex medical or nursing case management training/certification preferred.
