For over 125 years Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children youth adults seniors and those struggling with mental Housing provides housing and mental health services to those with documented history of homelessness substance abuse and chronic mental illness. Counseling case management substance abuse rehabilitation wellness socialization community integration and crisis management services are available for the OF THE JOBThe Housing Wellness Coordinator works as an integral member of Supportive Housing Programs for seniors. The Housing Wellness Coordinator is responsible for providing a range of on-site wellness services that meet the behavioral health and social needs of residents. The Housing Wellness Coordinator is responsible for meeting & assessing building tenants and developing & implementing appropriate and effective communal and individual wellness activities that support residents as they age in place. Under the supervision of a licensed clinician the Housing Wellness Coordinator will provide in depth supportive counseling and groups to residents. The goal is to support residents in maintaining permanent affordable housing. The Housing Wellness Coordinator will play an integral role ensuring that residents can age in place with Housing Wellness Coordinator ensures that the Agency Mission is met and best practices are upheld in our work with the buildings residents. The Housing Wellness Coordinator reports directly to the Clinical Director for day to day operations and social service AND RESPONSIBILITIESConduct and prepare assessments case logs documents case notes care plans for and maintain contacts with families social service support agencies and health residents for their needs and develops and implements service counseling behavioral interventions restorative services assistance in negotiating community activities and crisis management residents in developing meaningful service plan goals and provide supports to assist residents in achieving their wellness goals and activities with the input of residents and other and promotes wellness the development of appropriate wellness educational and social programming is consistently taking care for targeted critical/high need resident Licensed Practical Nurse with medication managementWork through team approach in partnership with building property management and maintenance staff to identify and respond to residents in positive advocacy and helps resident self-advocate with community service documentation both paper and electronic forms in a timely ongoing active outreach and creative engagement of to emergency situations on site and provide crisis contact and follow-up with service providers put in place for residentssuch as Adult Protective Services and/or outpatient treatment adhere to and enforce all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintain compliance with HIPAA residents to face to face meetings as requested by the staff meetings agency meetings workshops classes & in-service trainings on a regular basis to maintain current information on eligibility requirements for all basic other duties as FOR EDUCATION/CERTIFICATIONS/LICENSESBachelors Degree in Human Services Sociology Psychology Social Work or related fieldSPECIFICATIONS FOR EXPERIENCE AND TRAININGTwo years experience working in housing mental health senior and/or elderly populations or related organizational verbal & written communication literate including experience with all Microsoft Office software packages local area networks and the to work independently as well as part of a -Lingual Chinese/Korean/Spanish/English FOR PHYSICAL REQUIREMENTSRegularly required to talk hear walk stand & to climb stairs if to operate a computer keyboard mouse & office to read printed materials and computer to work flexible hours (evenings/weekends) as to travel throughout the five boroughs as to lift up to :We offer competitive salary and excellent benefits including:* Generous time off (Vacation/ Personal Days/ Sick Days/ Paid Holidays annually)* Medical* Dental* Vision* Retirement Savings with Agency Match* Transit* Flexible Spending Account* Life insurance* Public Loan Forgiveness Qualified Employer* Training Series and other additional voluntary more information on our organization please visit our website at: EOE/AA.
