As a Spiritual Coordinator youll contribute to our success in the following ways:

• Conduct initial and ongoing spiritual assessments to identify the needs beliefs and preferences of hospice patients and families.
• Develop individualized spiritual care plans and collaborate with the interdisciplinary team to integrate spiritual needs into overall patient care.
• Provide spiritual guidance counseling and emotional support while respecting each patients cultural and religious beliefs.
• Support patients and families with end-of-life concerns including discussions around meaning faith forgiveness and personal values.
• Provide bereavement support to families before and after a patients passing.
• Educate patients families and staff on spiritual care and maintain accurate timely documentation of services provided.

What is Required

• Bachelors or Masters degree in divinity Theology Pastoral Care or a related field
• 2 years of experience working at a hospice counseling or ministry environment
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations
