As a Registered Nurse Case Manager youll contribute to our success in the following ways:
- Assess hospice patients and families to identify physical psychological social spiritual and environmental needs.
- Monitor patient condition and promptly communicate significant changes concerns or safety issues to the physician and care team.
- Educate and support patients families caregivers and facility staff in following the interdisciplinary Plan of Care.
- Provide education on hospice philosophy pain management and symptom management to staff in nursing homes hospitals assisted living facilities and other care settings.
- Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
What is Required
- Current State license as a Registered Nurse.
- Graduate of an accredited Certificate Diploma Associate or Baccalaureate School of Nursing
- 1 year of experience as a nurse in a clinical care setting.
- Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
- Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
- Experience in hospice or home health environment is preferred
