Hospice Palliative Nurse Practitioner

VNA is looking for a caring and compassionate Nurse Practitioner to join our caring and compassionate Hospice/Palliative Care team. Responsibilities for this position include clinical practice education consultation and research. This position works interdependently with the multidisciplinary team guiding members of the multidisciplinary team (e.g. RNs LVNs HHAs PTs OTs STs MSWs nursing students and newly employed RNs) as appropriate and supervises patient plans of care including education on Hospice Benefit as appropriate. The NP may perform home visits when physicians/NP level visit is needed for patients in absence of their primary physician of record. Salary Range: $76.53 $83.22 Hour Status: Full-time Benefits: Medical Dental Vision Life Insurance 403(b) Education/Experience/Qualifications: Possession of a license to practice as a Nurse Practitioner in the State of California. Current CPR certification. Masters degree or equivalent certification requires a Furnishing License and DEA Certificate or obtain within one year of placement in position. Knowledge of Medicare regulations. Excellent knowledge of fundamentals of pain and symptom management. Excellent interpersonal and communication skills. Fluent in Spanish including medical terminology preferred. Demonstrated organizational skills. Must have the ability to establish and maintain cooperative relationships with professionals and lay groups. Ability to work collaboratively as part of an interdisciplinary group. Strong commitment to the interdisciplinary team approach to care.
