As a Medical Social Worker (MSW) youll contribute to our success in the following ways:
Assess patients to identify psychosocial financial environmental and safety needs.
Complete initial social work evaluations and reassess patient needs during follow-up visits.
Develop and update individualized care plans based on patient needs goals and physician orders.
Communicate changes in patient condition concerns or safety issues to physicians and the care team.
Connect patients and families with appropriate community resources and support services.
Counsel and educate patients and families to help address social emotional and care-related needs.
Coordinate with the care team to support continuity of care and effective discharge planning.
Provide compassionate high-quality social work services that support positive patient outcomes.
What is Required
Current State License as a Social Worker
Masters Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
1 year of social work experience in a healthcare setting
Social work skills as defined as general social work standards
Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
