Were hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships schedule stability and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance.

Youll step into a field-based hospice role providing direct patient care in homes and care facilities throughout Cape Cod. Youll independently manage your patient caseload while collaborating with an experienced interdisciplinary team dedicated to providing exceptional end-of-life care. Your territory will include Barnstable Bourne Cape Heritage Cape Regency and surrounding Cape Cod communities.

Manageable census of approximately 20 patients
Additional compensation for rotating weekend on-call coverage
$600 for weekend on-call coverage
Additional $100 per routine on-call visit
Additional $150 per on-call admission

What makes you a great fit

You hold an active Massachusetts RN license
You have at least one year of nursing experience
Hospice home health ICU or oncology experience is preferred
You value compassionate patient-centered end-of-life care
Youre comfortable independently managing a field-based caseload
You work well as part of an interdisciplinary healthcare team
Youre comfortable traveling throughout the Cape Cod territory

Compensation & Benefits

Base salary: $104000$114400 annually
Weekend on-call: $600 per weekend
Routine on-call visits: Additional $100 per visit
On-call admissions: Additional $150 per admission
