The admission visit is the moment a family stops feeling alone. Done well it changes everything that follows. We are looking for an experienced RN who can walk into a hard room explain hospice with clarity and compassion and give families confidence in the care ahead.

What Youll Do

• Perform hospice consultations and admissions assessing eligibility and appropriateness of care
• Educate patients and families on hospice services and patient rights with patience and honesty
• Communicate with physicians to secure admission orders medications and recertifications
• Participate in the Interdisciplinary Team Conference and plan of care review every 14 days
• Support documentation review to ensure patients continue to meet hospice qualifying criteria
• Provide clinical supervision competency evaluations and orientation support for nursing staff and CNAs
• Contribute to our QAPI program and to in-service education for staff and community partners
• Carry a part-time case load as assigned

What Were Looking For

• Active RN license in Idaho
• Two or more years of nursing experience with hospice or home health experience strongly preferred. If you have worked in hospice home health palliative care oncology or long-term care we want to hear from you.
• Expertise in pain and symptom management and end-of-life care
• Working knowledge of Medicare Medicaid state and accreditation requirements
• Strong clinical documentation and communication skills
• Valid drivers license vehicle insurance and reliable transportation
• Schedule flexibility local travel and participation in the on-call rotation
