Elara Caring is seeking a compassionate and motivated Home Health Physical Therapist Assistant (PTA) to provide personalized one-on-one therapy under the direction of a licensed Physical Therapist. Youll enjoy the independence of working in the field while receiving support from an experienced collaborative clinical team.

• Provide skilled physical therapy services according to the treatment plan established by the supervising Physical Therapist.

• Help patients improve mobility strength balance and overall function.

• Review the Plan of Care before each visit and follow physician orders and established clinical guidelines.

• Monitor patients progress and promptly communicate changes in condition to the Physical Therapist.

• Teach patients and caregivers how to safely complete prescribed home exercise programs.

• Provide instruction on the safe use of walkers canes wheelchairs prosthetics orthotics and other assistive devices.

• Accurately document visits patient progress and response to treatment.

• Follow all state practice requirements and organizational policies.

• Collaborate with the Physical Therapist and interdisciplinary care team to support positive patient outcomes.

• Graduate of an accredited Physical Therapist Assistant program.

• Current unrestricted PTA license in the state of practice.

• At least six months of PTA experience preferred.

• Home health experience preferred.

• Reliable transportation a valid drivers license and current automobile insurance.

• Ability and willingness to travel throughout the assigned territory.

• Sit stand bend lift and move up to 50 pounds including positioning or transferring patients and moving equipment
