Position Summary

The Speech Pathologist provides onetoone patientcentered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach collaborating with physicians nursing staff social workers and home health aides.

Candidates must be highly motivated detailoriented organized technologically proficient and comfortable working autonomously.

Responsibilities

• Provide individualized speech therapy services in patients homes
• Deliver patientcentered compassionate care
• Collaborate with interdisciplinary team members to support comprehensive care plans
• Utilize advanced and easytouse clinical technology
• Maintain accurate clinical documentation
• Demonstrate flexibility strong communication skills and professional accountability

Minimum Qualifications

• Meets educational and experience requirements for Certificate of Clinical Competence in Speech Pathology (CCCSLP) from ASHA
  OR
• Meets educational requirements and is currently completing the supervised experience required for certification registration or licensure in the State of Illinois (if applicable)
