Ascension Living PACE - Michigan is seeking to add a new Geriatric Nurse Practitioner to our team in Flint Michigan

This is an exceptional opportunity for a motivated Nurse Practitioner to join an employed position within a busy well-respected practice.

• Join an established team of 3 Physicians and 2 Apps
• Call structure: 1:5 weeks
• Practice setting: Outpatient practice caring for a panel of geriatric patients

At Ascension Living PACE Michigan we believe that geriatric care should be deeply relational not transactional. Located in the heart of Flint our team is comprised of a tight-knit collaborative groupof Physicians and APPswho prioritize a culture where every voice is heard and valued. We offer a truly sustainable practice model: with a manageable caseload of approximately 80 patients and a focused daily volume of 26 visits you are afforded the meaningful time required to build genuine longitudinal relationships with your patients.

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Practitioner obtained prior to hire date or job transfer date required.
• Registered Nurse obtained prior to hire date or job transfer date required.

Education:

• Masters degree of Nursing required.
