Communication: Exceptional verbal and written communication skills for interacting with families service providers and multidisciplinary teams
Detail Oriented: Meticulous attention to documentation compliance requirements and individualized service plan details
Organizational Skills: Superior ability to manage multiple cases prioritize tasks and maintain comprehensive records
Primary Responsibilities
Serve as the primary service coordinator for an assigned caseload of families with children eligible for early intervention services
Conduct initial and ongoing assessments to identify family needs priorities and resources
Coordinate and facilitate multidisciplinary team meetings including Individualized Family Service Plan (IFSP) development and review sessions
Ensure timely delivery of all required early intervention services as outlined in each childs IFSP
Monitor service implementation and make adjustments as needed to meet changing family circumstances and child development
