Salary: $100000 - $107000 per year

Catholic Charities of Baltimore Caritas House is seeking a Delegating Nurse Manager to provide nursing leadership and collaborates with an interdisciplinary team to support the medical health and well-being of individuals residing in residential programs. This role provides nursing oversight and direction to direct care professionals in implementing the medical components of individual care plans and ensures care is delivered in accordance with established nursing standards care plans and applicable requirements. The Delegating Nurse also supervises the Part-Time Delegating Nurse providing guidance support and oversight in the delivery of delegated nursing services.

The work schedule is Monday through Friday 8:00 a.m. to 4:30 p.m. This position requires flexibility to provide on-call support as needed. As this is a 24-hour facility the employee may be contacted for emergencies outside of regular business hours including evenings nights and weekends.

JOB DUTIES & RESPONSIBILITIES:

• Provides nursing leadership for residential programs by guiding the medical components of individual care plans and promoting safe person-centered care for assigned residents.
• Manages Delegating Nurse responsibilities and supervises the Part-Time Delegating Nurse by providing clinical direction coaching performance feedback and oversight to ensure delegated nursing services are completed in accordance with established standards and applicable requirements.
• Oversees case management for assigned residents by assessing health needs coordinating medical services monitoring changes in condition and ensuring timely follow-up with providers and interdisciplinary team members.
• Develops revises and monitors nursing diagnoses care plans goals and interventions to address identified health needs and support positive resident outcomes.
• Delegates nursing functions to licensed and unlicensed direct care professionals and monitors medication administration delegated tasks and staff competency for the assigned caseload.
• Trains supports and advises direct care professionals and Certified Medication Technicians on care plan requirements medical procedures treatments and nursing services to promote safe and consistent implementation.
• Completes the required 4-hour recertification course and provides training to CMTs to support timely renewal of CMT certification.
• Coordinates with residents families providers direct care professionals and interdisciplinary team members to clarify care needs connect resources resolve service barriers and support continuity of care.
• Completes required nursing assessments evaluations reviews reports and health documentation accurately consistently and within established timeframes.
• Reviews documentation care delivery concerns and data quality issues to identify trends correct gaps and support compliance with Agency expectations nursing standards and regulatory requirements.
• Makes appropriate clinical decisions within scope of practice and promptly escalates significant health changes service barriers safety concerns or compliance risks to the Director and appropriate stakeholders.
• Provides or coordinates 24/7 on-call nursing coverage to ensure timely clinical guidance response to urgent health concerns and continuity of care for residents.
• Advises the Director on resident health needs nursing services staffing considerations and opportunities to strengthen care coordination compliance and program performance.
• Performs other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS:

• Associate degree in nursing from an accredited nursing education program.
• Current Delegating Nurse certification maintained in accordance with Maryland Board of Nursing requirements and all applicable state regulations.
• Minimum of three years of professional nursing experience including at least one year providing care in a long-term care dementia care acute care or comparable clinical setting.
• Working knowledge of Medicare skilled-level coverage criteria rehabilitation services and related documentation and care coordination requirements.

Certification/Licensure:

• Registered Nurse (RN) license in good standing with the Maryland Board of Nursing (MBON).
• Current CPR and First Aid certification

• Requires proficiency in using computer systems and software to perform job function including but not limited to basic Windows PC web browsing (i.e. Chrome Internet Explorer etc.) and Microsoft O Knowledge of other Microsoft Office applications such as Word Excel PowerPoint Teams and OneDrive is preferred.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

• No remote or hybrid work availability based on core and essential functions of the position.
• Flexibility is required to work beyond standard hours including weekends and holidays and to provide 24/7 on-call services as needed.
• Adjusting or moving objects up to 30 in all directions which may include lifting carrying pushing pulling or repositioning items as necessary to perform job duties.
• Engaging in communication with others to exchange information which may involve speaking listening writing or using electronic communication methods to ensure clear and effective understanding. This includes both verbal and non-verbal interactions with colleagues clients or external parties.
• Repetitive motions involving the wrists hands and/or fingers including tasks such as typing data entry or handling small objects. These repetitive activities may occur throughout the workday and require attention to ergonomics to ensure comfort and minimize strain.
