EMPLOYMENT TYPE: Full-Time 40 hours/week

SCHEDULED HOURS: Monday-Friday 8:00AM-8:00PM availability hours flexible some evenings required

ABOUT THE PROGRAM: Credible Messenger Program

The Credible Messenger program is a non-clinical care support and case management program that aims to assists high-risk justice-involved youth and their families living in Hartford with their identified needs related to behavioral health substance use medical care housing education employment legal matters transportation benefits family support and other activities of daily living. The goal of the program is to reduce re-arrests for the youth while connecting them to prosocial supports and education within their community. The Credible Messenger position works directly with the youth on their goals as well as while completing administrative programmatic triage and documentation responsibilities in accordance with program and contract expectations.

The Credible Messenger position is a good fit for those who:

  • Have lived experience with the justice system and/or who have personal roots within the city of Hartford CT. Knowledge and familiarity with the neighborhoods of Hartford and the challenges and needs of youth living in this area is preferred.
  • Have experience with and enjoy building trust and rapport with high-risk justice-involved inner-city youth and their families.
  • Have knowledge of local resources available to youth in the city of Hartford
  • Have experience with collaborating with Juvenile Probation Officers or the court system
  • Enjoy working in the community and in clients homes as opposed to an office-based position
  • Prefer a flexible work schedule with non-traditional hours including some evenings and being available to youth and their families outside of traditional hours.
  • Are motivated and take initiative to support and advocate for disadvantaged youth.
  • Who are creative and persistent in their strategies to engage reluctant youth in the program

Duties & Responsibilities

  • Coordinate intake engagement and ongoing services for youth referred by CSSD/JBCSSD in accordance with program contract requirements and the Individual Service Plan.
  • Serve as a liaison between CHR CSSD/JBCSSD clients families natural supports and community providers to support communication service access treatment planning and discharge planning.
  • Link clients and families to community supports and resources including behavioral health substance use medical care housing benefits legal services education employment and other identified needs.
  • Assist clients with appointment scheduling transportation coordination and other practical supports that promote engagement and reduce barriers to care.
  • Maintain accurate timely documentation of services contacts referrals and communications in compliance with agency program and contract standards.
  • Participate in supervision team meetings trainings and other meetings as assigned.
  • Perform other duties as required.

QUALIFICATIONS:

Education: High School Diploma or equivalency program (GED)

Experience: Credible Messengers will come from and live in the communities of the youth to be served shall have lived similar relevant experiences of the youth and have transformed their own lives.

Licensure/Certification/Registration: Valid drivers license required.

Training: Will attend all required program trainings for CHR and CSSD

COMPENSATION*:

$19.2850/hr min-$26.9735/hr max

*Actual rates within this range are determined at the time of offer and are based on relevant experience as well as adherence to our internal equity policy. (Please note for part-time positions this range would be pro-rated based on hours worked)
