How youll contribute

As supervised and delegated by a registered nurse provides direct patient care to patients in accordance with applicable scope and standards of practice.

Essential Functions:

• Under the direction of an RN contributes to the assessment of patients by collecting data for analysis by an RN.
• Coordinates PCMH training activities.
• Completes data analysis to drive efforts to meet quality benchmarks.
• Serves as a role model in PCMH by developing educating and training clinic staff/providers on quality measures PCMH concepts and reporting.
• Performs activities related to the collection processing and analysis of quality data to assure accuracy and reliability.
• Identifies and prioritizes quality measure performance needing improvement.
• Assist with orientation and training of personnel and students.
• Participates and attends meetings as required and/ or assigned.
• Performs other related duties as assigned or requested.
• Prepares monthly reports and as requested.
• Review and comply with LifePoint Code of Conduct and all relevant Company and Division policies and procedures.

What were looking for

Applicants should have a high school diploma or equivalent required. Additional requirements include:

• Active North Carolina LPN license (single-state) or compact required or Paramedic / EMT. If relocating licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.

• Basic Life Support certification is required within 30 days of hire

• 1 year of Electronic Medical Records experience

• 1 year of Patient Centered Medical Home (PCMH) experience

• 1 year of quality review/ quality management experience

• 3 years of ambulatory care experience (Preferred)

• Requires critical thinking skills decisive judgment and the ability to work with minimal supervision.

• Must be able to work in a stressful environment and take appropriate action.
