Job Title: CNA/DSP (Certified Nursing Assistant / Direct Support Professional)

Job Summary:
The CNA/DSP provides compassionate person-centered care and support to individuals with disabilities special needs or health-related challenges. This role assists with daily living activities promotes independence ensures health and safety and supports individuals in achieving their personal goals within home and community settings.

This Position has specific hours: 1st shift 9:30a-11:30a- 2nd shift 1:30p-7:30p

Key Responsibilities:

• Assist individuals with personal care including bathing dressing grooming and toileting.
• Support daily living activities and encourage independence.
• Monitor and report changes in health behavior or well-being.
• Assist with medication reminders as permitted by policy.
• Maintain accurate documentation and records.
• Provide companionship and emotional support.
• Ensure a safe clean and supportive environment.
• Transport and accompany individuals to appointments and community activities as needed.

Qualifications:

• Current CNA certification (if required by position/state regulations).
• Experience working with individuals with disabilities or special needs preferred.
• Strong communication and interpersonal skills.
• 18 yrs or older
• Ability to work independently and as part of a team.
• Valid drivers license and reliable transportation preferred.
• Valid Car Insurance
• CPR/First Aide preferred

Physical Requirements:

• Ability to lift transfer and assist individuals as needed.
• Ability to stand walk bend and perform physical caregiving tasks throughout the shift.
