CNA LTC

Job Summary:
We are seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide direct patient care and support to residents in a long-term care facility. The CNA will assist residents with activities of daily living maintain patient comfort and safety and work closely with nurses and other healthcare professionals.

Key Responsibilities:

• Provide personal care and assistance with activities of daily living (ADLs) including bathing dressing grooming toileting and feeding.
• Assist residents with mobility transfers positioning and ambulation.
• Monitor and report changes in residents condition behavior or vital signs to the nursing staff.
• Measure and record vital signs intake/output and other required observations.
• Maintain a clean safe and comfortable environment for residents.
• Assist with meal service and ensure residents receive appropriate nutrition and hydration.
• Follow infection-control safety and facility policies.
• Provide emotional support and maintain respectful compassionate relationships with residents.
• Assist nurses with patient-care activities within the CNA scope of practice.
• Accurately document care provided and communicate effectively with the healthcare team.
• Follow residents individualized care plans and facility procedures.

Requirements:

• Current and active CNA certification/license in the applicable state.
• BLS certification preferred/required based on facility requirements.
• Previous experience in Long-Term Care Skilled Nursing Nursing Home or Geriatric care preferred.
• Strong communication and interpersonal skills.
• Ability to work as part of a multidisciplinary healthcare team.
• Ability to stand walk bend lift and assist residents with transfers and mobility.
• Compassionate reliable and patient-focused approach to care.

CNA: Basic duties including assistance with ADLs bathing and meal set up. We have 2 floors. May float between the two floors.
