Job Summary
We are seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide direct patient care and support in a Long-Term Care (LTC) setting. The CNA will work closely with nurses and other healthcare professionals to ensure residents receive safe respectful and high-quality care.
- Provide direct care and assistance to residents with activities of daily living (ADLs) including bathing grooming dressing toileting and feeding.
- Assist residents with mobility transfers positioning and ambulation while following proper safety procedures.
- Monitor and report changes in residents condition behavior appetite or vital signs to the nursing staff.
- Take and accurately document vital signs and other assigned observations.
- Assist with maintaining a clean safe and comfortable environment for residents.
- Reposition residents as needed to promote comfort and prevent skin breakdown.
- Support residents with personal hygiene and maintain dignity and privacy at all times.
- Assist nurses with routine patient-care activities within the CNAs scope of practice.
- Document care provided accurately and in a timely manner.
- Follow facility policies infection-control procedures and safety regulations.
- Communicate effectively with residents families nurses and other members of the healthcare team.
- Provide compassionate emotional and social support to residents.
- Maintain confidentiality and follow HIPAA requirements.
- Active CNA certification/license in the applicable state.
- Current BLS certification.
- Previous experience in Long-Term Care Skilled Nursing Facility (SNF) Nursing Home or Rehabilitation preferred.
- Ability to provide safe and compassionate resident care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a healthcare team.
- Ability to work assigned shifts and maintain reliable attendance.

The CNA will work in a long-term care environment providing continuous care and support to residents with varying levels of medical and personal-care needs. The role may require prolonged standing/walking frequent bending lifting transferring and assisting residents with mobility.
This position works in a hospital or long term care facility and is responsible for providing direct patient care through the performance of a variety of nursing support tasks in accordance with established methods and procedures. The position may work on rotating shifts and in a variety of care units (e.g. SNF ICF medical surgical etc.). The position may also provide home care for patients.
