As a member of the Leadership Team the Clinical Program Manager provides operational oversight to the programs projects and teams assigned to them that provide community-based services as well as contributes to the evaluation of the several grants that fund the Clinical Programs Department. This position champions the care continuum for Prisma Community Care services executing strategies that promote access linkage and care collaboration with the department director this role will work heavily with various databases and systems to identify growth opportunities and the success of projects and programmatic initiatives. Specific programs and projects will be assigned to the Clinical Program Manager and may change based on organizational and departmental need. The Clinical Program Manager position demands a high level of cultural competency to serve the unique needs of the 2SLGBTQIA community communities of color and other marginalized and/or vulnerable populations.
The Clinical Program Manager supports the Prisma Community Care mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color 2SLGBTQIA and Queer individuals and those affected by HIV.
Essential Functions
Manage oversee and as appropriate develop the programs and/or projects that are assigned by the department director ensuring success and sustainability in departmental initiatives and strategies as related to the care continuum.
Generate and analyze data reports on program productivity and health informatics ensuring timely and accurate reporting to funding entities as well as synthesizing data to identify growth areas and strategize opportunities for improvement.
Ensure all client encounters and activities are documented in system databases and EMRs within three (3) business days in accordance with internal and state policy.
Oversee staffing and their performance to ensure operational efficiency to include conducting regular meetings with team members to support development and success leading staff to ensure high quality service delivery aligned with grant requirements and best practices as it pertains to clinical programs.
Manage the successful completion of grant deliverables for the programs teams and projects assigned ensuring the awards remain in good standing and continually meeting objectives and goals.
Contribute to the training and education process for all team members of the organization regardless of role to promote basic understanding of programmatic functions and patient experience excellence.
Collaborate with internal teams to identify and address barriers to patient engagement implementing solutions to improve access linkage retention and quality of care.
Support the development and execution of Clinical Programs that align with Prismas mission to expand sexual health and wellness services.
Oversee the systems and databases that are used to report health data and track programmatic enrollments and engagements to enhance all areas of the care continuum as it applies to the specific program assignment with the intention to ensure processes are optimized and used efficiently.
Collect interpret and analyze assigned health data to provide insights and reports to leadership.
Manage and administer assigned health data related software or databases.
Monitor program budgets and reconcile expenditures to ensure compliance with grant-funded assistance programs with support from the department director.
Provide direct collaboration with local health departments regarding communicable diseases and infection control as it relates to the Clinical Programs Department in addition to ensuring patients are successfully linked to care.
Research adapt and restructure resources and referrals that specifically relate needs to the 2SLGBTQIA community and their ability to access and maintain care.
Stay informed engaged and current with emerging research trends and best practices in the field of gender identity intersectionality and wellbeing of all.
Work collaboratively with other teams to further develop and evolve the needs of the department and organization including but not limited to policy reformation cross-training and identifying opportunities for growth and improvement within teams.
Work directly with the department leadership with other program-related tasks.

Minimum Qualifications
Bachelor of Science degree or four (4) years of equivalent experience in healthcare or public health with a high school diploma or equivalent.
A minimum of three (3) years of related experience in a medical office or related healthcare setting and a minimum of two (2) years of leadership or supervisory experience.
Demonstrated understanding and application of collecting interpreting and analyzing health data.
Familiarity with federal and state programs grants and program management.
Demonstrated ability to establish rapport and maintain effective communication with culturally diverse populations from a wide range of life circumstances and backgrounds.
Demonstrate a sound understanding of gender identity gender expression gender presentation sex and sexual orientation etc. This includes a strong grasp of local and national resources (medical mental health social etc.) for those that are transgender or gender non-conforming.
Proficient in data analysis software applications and competent in using electronic medical records and information systems in providing effective documentation of patient care with a full understanding of HIPAA and protecting patient confidentiality.
Demonstrate an understanding of basic medical terminology.
Highly organized detail-oriented self-starter with excellent problem-solving skills and the ability to assess organizational needs independently and collaboratively.

Preferred Qualifications
Programmatic experience related to the empowerment of sexual health and wellness.
Demonstrated understanding of sexual health and wellness and its correlation to HIV PrEP PEP and gender-affirming care.
Demonstrated skills knowledge and experience in the deployment and execution of outreach or community engagement activities.
Established strong relationships with community healthcare organizations and established knowledge of HIV/ STI prevention in 2SLGBTQIA communities.
Possess a valid drivers license insurable driving record and access to a reliable personal vehicle for work-related activities.
Experience working with data in PowerBI.
Bilingual in Spanish and English demonstrating the Listening & Speaking or Medical Assessment skill
Experience with eClinical Works (eCW).
Environmental Factors and Conditions/Physical Requirements
Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
Travel outside of the Phoenix area approximately 10% of the time.
