Compensation Range: $21/hr - $24/hr commensurate with experience

Overview:
The BEWâ(BeâIntoâHealth forâWomen)ââProject is a partnership between Housing Works and NYC DOHMH supports Black and Latina women living with HIV in reengaging in HIV care and essential support services. The Clinical Care Manager works with an interdisciplinary team to assess client needs coordinate care develop health goals and promote retention in care and self-management. The ideal candidate has experience serving high-risk populations and strong cultural competency working with diverse gender identities and communities.
Responsibilities:
- Facilitateâclient enrollment in BEW program including assisting withâintake outreach/engagementâand eligibilityâverification.â
- Conduct initial needs assessments andâcollaborateâwith clients on establishingâachievableâcare plan goals based onâidentifiedâneeds.â
- Conduct reassessment and update plans of care at regular intervals.â
- Verify engagement in HIV treatmentâprovideâtimelyâreconnection to care.â
- Collaborate withâthe interdisciplinaryâteam on the delivery of individual and groupâstructuredâhealth education sessions.â
- Provide direction and support to the Client Navigator in the delivery of enhancedâclient navigation servicesâincludingâassistanceâwith access to health care socialâservices entitlements orâotherâservices that reduce barriers to engaging in healthcare.
Requirements:
- Master Social Worker (MSWLMSW)âor BA/BS with no less than 5 years care management or navigation experience.
- Cultural competency with Black andâLatina Cisgenderâtransgender on-binary orâgenderqueer women.
- Prior experience with completing assessments and developing care plan goals.
- Experience with documentation inâelectronicârecords
- Abilityâto work in a team management and interdisciplinary system
- Understanding of HIV/AIDS and its impact onâhomelessness substanceâuse and mental illness
- Bilingual (English/Spanish English/Haitian-Creole) preferred but notârequired.
