Job Summary

The Community Health Worker (CHW) Supervisor provides day-to-day supervision guidance and support to Community Health Workers (CHWs) within the Family Community & Beyond Program. The supervisor ensures high-quality service delivery to pregnant postpartum and parenting families while maintaining compliance with New York State Department of Health (NYS DOH) requirements program goals grant deliverables and hospital position combines leadership staff development quality improvement community engagement and direct operational oversight to ensure families receive comprehensive culturally responsive trauma-informed care.

  • Provide daily supervision and support to Community Health Workers.
  • Conduct regular individual supervision meetings.
  • Assist staff with complex client situations and problem-solving.
  • Monitor staff productivity caseloads and performance expectations.
  • Complete performance evaluations and provide ongoing coaching.
  • Develop improvement plans when necessary.
  • Assist with onboarding orientation and training of new employees.
  • Promote teamwork accountability and professional development.
  • Ensure implementation of program goals and grant deliverables.
  • Monitor compliance with NYS DOH PICHC requirements.
  • Assist with scheduling community outreach activities workshops and educational events.
  • Coordinate CHW coverage for hospital and community events.
  • Maintain adequate program supplies and educational materials.
  • Assist with planning Baby Safety Showers workshops parenting classes and community outreach activities.
  • Review CHW caseloads regularly.
  • Ensure timely client follow-up.
  • Monitor referrals and linkage to services.
  • Support staff with high-risk or challenging client situations.
  • Promote client-centered culturally competent trauma-informed care.
  • Ensure confidentiality and HIPAA compliance.
  • Review documentation for completeness and accuracy.
  • Monitor data entry within required databases.
  • Ensure timely completion of client assessments care plans referrals follow-up documentation monthly reports.
  • Assist with quality assurance reviews.
  • Monitor program performance indicators.
  • Develop and maintain relationships with WICOB/GYN Clinics Pediatric providersCommunity-Based OrganizationsGovernment agencies Social service partners.
  • Represent the program at community meetings.
  • Promote collaborative referral processes.
  • Assist with the development of educational materials.
  • Support implementation of evidence-based health education.
  • Participate in community outreach events.
  • Promote maternal infant and family health initiatives.
  • Assist in coordinating workshops and educational programming.
  • Monitor program outcomes and quality metrics.
  • Identify opportunities for program improvement.
  • Assist with grant reporting and site visit preparation.
  • Participate in continuous quality improvement initiatives.
  • Ensure compliance with hospital policies and accreditation standards.

    Position Requirements:

    • This position requires a minimum formal education of Associates Degree in a related field and a minimum of three (3) years of relevant experience in community health public health social services or a related healthcare setting. A combination of education and experience may be considered.
    • Minimum three (3) years of experience as a Community Health Worker or in community-based health programs.
    • Minimum one (1) year of leadership supervisory or team lead experience preferred.
    • Experience working with maternal and child health populations strongly preferred.
    • Experience working with diverse and underserved communities.
    • Experience coordinating community outreach programs.
    • Strong leadership and supervisory skills

    • Cultural humility and sensitivity
    • Knowledge of maternal and infant health
    • Understanding of social determinants of health

    • Proficiency in Microsoft Office (Word Excel Outlook Teams).

        Preferred Qualifications:

        • Certified Community Health Worker (CCHW)
        • Bilingual English/Spanish strongly preferred
        • Experience with NYS DOH grant-funded programs
        • Experience with quality improvement initiatives
        • Experience with electronic documentation systems
        • Experience supervising multidisciplinary teams
