Salus Hospice is seeking a compassionate Chaplain/Bereavement Coordinator to provide spiritual care emotional support and grief counseling to patients and families throughout the hospice journey.

• Complete spiritual assessments within required timeframes including spiritual and emotional factors related to the patients condition.
• Provide individual and family support through spiritual needs assessments and counseling.
• Attend interdisciplinary team (IDT) meetings regularly.
• Develop implement and revise care plans based on changing patient and family needs.
• Provide spiritual care resources and referrals to clergy representing diverse religions and faith traditions.
• Provide bereavement support and grief counseling to families before and after a patients passing.
• Coordinate bereavement follow-up services and maintain accurate documentation.
• Collaborate with the hospice team to ensure compassionate patient-centered care.

• Graduate of an accredited seminary or school of theology appropriate certification in hospital or pastoral ministry or two years of hospice experience preferred.
• Experience in chaplaincy geriatrics hospice end-of-life care or supporting patients and families facing life-threatening illnesses and death.
• Ability to remain open flexible and respectful while providing ecumenical spiritual care.
• Strong communication listening and interpersonal skills.
• Reliable transportation valid drivers license and current auto insurance.
• Willingness to travel regularly to patient locations.
