Our rapid growth is providing exciting opportunities in all areas of our Case Manager Position Summary:The CaseManagerperforms case management-related functions to coordinatefacilitateand maintain care forthe residents of Venture House and the Apartment Treatment programin Madison Case Manager Responsibilities:Completes assessment of housing and housing related with Venture House and Apartment Treatment staff and multi-disciplinary treatment team to establish individualized service plans which meet the clients and maintains effective relationships with other service providers: e.g. Madison County DSS SPOA Coordinator PROS Care Coordination as program liaison and primary contact for other service providers as it relates to clients coordination of current understanding of the Office of Mental Health regulations mental health disorders and its treatments. Understands reciprocal relationships between substance abuse and mental individuals with linkage and referral to substance use disorder and mental health services where to and effectively handles emergencies which may require flexibility of time and area of all required documentation in timelines in accordance with program Case Manager Qualifications:Bachelors/Associates and one (1) year experience or HS Diploma and three (3) years related experience.
