HASA NY/NY III Scattered Site Housing Program: This program provides 50 units of scattered-site supportive housing to chronically homeless single adults who are persons living HIV/AIDS or are clients with symptomatic HIV who are receiving cash assistance from the City and who suffer from co-existing serious and persistent mental illness a substance abuse disorder or a Mentally Ill Chemical Abuse (MICA) disorder. Clients spouses and domestic partners can live there but no minor children adult children and friends are allowed to live with clients. HASA is the sole referral source for this program. Staff will provide case management and other supportive services to enable clients to achieve the skills and financial independence necessary for independent living.

What The Case Manager Does:

  • The Case Manager will be responsible forprovidingall contract funded activitiesas described below.
  • Case Manager will be responsible for completing all program documentation and entering all client servicesintoClient Track such as assessments/reassessments service plans and progress notes.
  • Refer and connect clients to needed services suchasmedical mental health substance use treatment legalaide entitlements etc.
  • Ensure clients are compliant with medical treatment and medication management adherence
  • Monitor clients progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits and document via progress notes.
  • Function as client liaison/client advocate with outside organizations regarding such matters as education healthcare housing legal issues etc.
  • Escort clients to appointments (educational medical social service etc.)
  • Assist clients in completing applications for benefits and entitlementsand monitors outcome of process.
  • Collect all required documentation and create client files.
  • Conduct a minimum one home visit per month.
  • Report any repair issues to the Housing Specialist.
  • Monitor clients living conditions.
  • Meet required monthly projections.
  • Provide all required information for the completion of monthly program reports.
  • Maintain clients charts always audit ready.
  • Other duties as assigned.

What are the Qualifications for the Position:

  • BachelorsDegree
  • Associatesdegree/high school diploma/GED withat least4years experience incase management or related socialservice field(housing homelessness mental health substance abuse etc.).

Minimum Experiences and Skills Required:

  • Relevant experienceas specified above preferably in a mid-sized (250 employees) organizationworkingwith persons with ahistoryof homelessness mental illness and/or substance abuse.
  • Goodwritten & verbal communication.
  • Computer literacy in Microsoft Office Suite.

Compensation: $53045. annually
When salary ranges are listed the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and includes but is not limited to experience education budget and/or program size internal equity skills and other factors that may be required for the position and organization.

Status: Full-Time
