Job Summary

SUMMARYThis position works as part of an Assertive Community Team that provides care and treatment for clients that are experiencing significant mental health/psychiatric symptoms who are demonstrating significant functional impairments and who may have co-occurring chemical dependency and or personality disorder. The Practitioner is viewed as a primary agent of hope and change and is required to work in full partnership with the client in his/her recovery process. The practitioner has a two focused level of responsibility: first for the care welfare treatment and safety of the clients as individuals and secondly for the facilitation of a work and a treatment culture that promotes trans-disciplinary team work professionalism cultural competency and adherence to best practice principles and interventions. The Practitioner has responsibility for client specific and program outcomes. Primary duties include assessment crisis assessment and management planning individual counseling skills teaching group work case management household management financial eligibility assistance and JOB FUNCTIONSTo perform this job successfully an individual must be able to satisfactorily perform each essential function listed below. Conducts over-all rehabilitation goal planning which may include living learning working social and spiritual assessments: mental health status review suicidal vulnerable adult & immediate needs functional living skill resource chemical dependency and vocational/ and summarizes findings with recommended treatment/rehabilitative activities and and monitors the individual treatment/rehabilitative the treatment/rehabilitative activities and crisis at the lowest level of and over-sees client financial needs living arrangement services and monitors transition and recovery support staff and client consultation when and monitors the civil commitment to Data Privacy Practices/ receives and writes information clearly and and interprets clinical assessments and crisis/treatment/behavioral plans with differential goals and progress notes summaries and and records medication administration objective verbal and written critical incident essential activities related to licensure welfare and service client specific and program in the 360-degree performance evaluation knowledge and skills based on client and program supervisory over-sight and personal other related duties and activities as requiredSUPERVISORY RESPONSIBILITIESNoneMinimum Knowledge and Skills required for the JobThe requirements listed below are representative of the knowledge skill and/or abilities required to perform the job. Education and Experience: Holds a bachelors degree in one of the behavioral sciences or related fields from an accredited college or university and has at least 4000 hours of supervised experience in the delivery of services to persons with mental illness; or holds a graduate degree in one of the behavioral sciences or related fields and has at least 2000 post BA hours of supervised experience in the delivery of services to persons with mental illnessCertificates Licenses and Registrations:This position requires only that license which may be required based on education and standards determined by educational/degree guidelines. All Practitioners are supported in gaining certification as a Certified Psychiatric Rehabilitation Practitioner. The agency encourages staff to seek license at the highest standard permitted by his/her education and Skills and Abilities:The Mental Health Practitioner/Case Manager must be able to problem-solve in a creative and independent fashion. Treatment interventions require that resolution must be sought that promotes the safety welfare care and treatment of the client(s). The Practitioner must be able to intervene in a crisis in a calm controlled and effective manner. On-call responsibilities require assessment and problem resolutionThe Mental Health Practitioner/Case Manager has authorization to act in accordance with agency policy and procedures and within the scope of the job description. Law requires clinical supervision and clinical over-sight. A Practitioner should seek appropriate levels of supervisory input as the need arises. The Practitioner reports to the Clinical Director. The Clinical Director determines performance competency jointly with the Administrator and with input from peers and hire this position requires demonstration of the ability to conduct functional assessments develop treatment plans provide crisis and behavioral management provide goal directed individual counseling facilitate groups teach skills provide case management problem-solve and communicate effectively orally and in writing. This position requires teamwork and the ability to work position requires demonstration through training and testing within a designated time frame of the following core competency areas: First Aide/CPR Data Privacy (HIPPA) Non-Violent Crisis Intervention Medication Monitoring/Administration Principles and Techniques of Psychiatric Rehabilitation (including Over-All Rehabilitation Goal Setting Functional Assessment Skills Teaching and Programming) Empathy/Motivational Training Working with Persons Experiencing Mania Hallucinations and Delusions and Relapse Prevention an Assertive Community Treatment service the inter-disciplinary team offers evidence-based treatment and best practices in several areas. This may result in the Clinical Directors need to balance the skills and expertise of the staff; and thus when seeking a new hire or in providing training the Practitioner may be required to have a specialized area of knowledge skill and or licensure. The Practitioner rotates on-call for emergency consultation and must be able to return to work on short notice as Requirements:75-100% TravelThis position involves the majority of work to be done in the community. The Practitioner is required to have a well-maintained car with current car insurance a safe driving record and valid drivers license. The employee must also maintain a clear criminal background study check (Rule 11) and sexual conduct questionnaire (MN Statute 148A). The Practitioner rotates on-call for emergency consultation and must be able to return to work on short notice as Requirements:Medium Work. Exerting up to 50 pounds of force occasionally and/or up to 30 pounds of force frequently and/or up to 10 pounds of force constantly to move Range: $65000-$67000
