Job Summary
Catholic Charities is working closely with the City of San Jose Mayorâs Gang Prevention Task Force to Case Management Services Outreach Services Weekly Support Groups and Pro-Social activities to youth according to Hot Spotâs identified on the City of San Jose BEST Contract. The case manager implements outreach and support to empower youth & their families in making healthy choices.
ESSENTIAL DUTIES & RESPONSIBILITIES:
ADMINISTRATIVE DUTIES :
âResearches and has a working knowledge of community resources
âProvides needs assessment to address through linkage and case management services
âAssists and links families to needed internal and external resources to promote overall well-being and improved mental health
âAssist families with service provider process navigation to ensure service obtainment
âEnsure that all required forms consents family plans legal documentation etc. are properly completed and kept up to date
âTransfer family and service documentation to electronic data systems as necessary
âManage and maintain a caseload of youth and families
âPrepare weekly and monthly clientele reports on Excel spreadsheets
âIn a timely manner commit to other administrative duties such as creating a weekly schedule maintaining reimbursement reports online timesheets and clientele weekly and monthly reports.
âMeet with direct supervisor on a weekly basis to discuss clients program adjustments required support etc.
PROGRAMMATIC DUTIES:
âProvide Pro Social Activities on Tuesday & Thursday evenings
âProvide transition services to integrate the youth back into school and to help the youth succeed once there.
âEstablish and maintain good working relationships with school administration community partners SJPD and Juvenile Probation Department.
âImplement program evaluation procedures such as pre and post surveys assessments and ISPâs.
âRepresent Children Youth and Family Development at community events.
âAssist team with coordination of services.
âProvide after hour support over the phone for clients in need.
âAdvocate for client in court school probation and âreferring-outâ circumstances.
âProvide a weekly 1-1 (School or Home Visit with each youth as needed.)
QUALIFICATIONS:
Education: BA preferred and/or extensive equivalent work experience/knowledge
(Equivalent education and experience that demonstrates the ability to perform the job may be considered.)
Bilingual is a plus.
REQUIRED KNOWLEDGE SKILLS AND ABILITIES EXPERIENCE:
Desire to work autonomously and in a team-setting.
Implements the attitude to balance administrative duties as well as working with families and the community at-large.
Case Management/Counseling experience in a community based setting;
Effective verbal and written communication skills;
Understanding and sensitivity of economic cultural issues in diverse communities;
Ability to work independently with minimal supervision;
Ability to meet deadlines;
Demonstrated computer literacy (MS Word Excel and Internet usage);
Fingerprinting and background check as required by the DOJ and DMV clearance;
TB test required
PHYSICAL EMOTIONAL and INTELLECTUAL REQUIREMENTS:
âExtensive knowledge of how to work with at-risk and gang impacted youth.
âKnowledge of asset-based youth development strategies
âAbility to collaborate with CBOâs & agencies working with at-risk gang related or gang impacted youth.
âExperience working with diverse populations.
âExperience working with youth in the juvenile probation system.
âFast paced diverse and dynamic environment
âThe service is sometimes physical requiring setting up for events which may require transporting or lifting cumbersome materials e.g. easels flip charts refreshments brochures chairs tables etc.
âMust be able to report to service site on a regular and reliable basis
âAble to deal with stressful situations
âMust be able to work a flexible schedule which includes day evening weekend and holiday hours as appropriate to coordinate and provide services and activities for families during non-traditional hours
âAble to travel between sites and to attend offsite events throughout the city as appropriate
PHYSICAL REQUIREMENTS:
âAble to use the telephone and computer (including use of mouse and keyboard)
âAble to drive
âAble to lift up to 25 lbs.
âAble to walk long distances
