As a Case Management Supervisor at Horizon you will.
- Supervise and support TCM and PRS staff through regular coaching training and performance management.
- Manage referral management and oversight
- Monitor caseloads service delivery productivity and documentation quality.
- Ensure compliance with CCBHC OMH OASAS Medicaid and agency standards.
- Review documentation and conduct quality assurance activities.
- Assist staff with complex cases care coordination and barriers to engagement.
- Collaborate with internal teams and community partners to ensure continuity of care and successful service outcomes.
- Participate in program development quality improvement initiatives and regulatory audits.
- Prepare reports and maintain program performance data.
- Service delivery to patients as needed.

What makes you a great candidate
We cant wait to learn more about you! Here are a few specifics of what youll need for the job:
- Bachelors degree in Social Work Human Services Psychology Rehabilitation Counseling or a related field required.
- 2 years of experience in behavioral health and vocational required.
- 2 years of supervisory or leadership experience required.
- Knowledge of behavioral health recovery-oriented care and trauma-informed practices.
- Understanding of Medicaid-funded services and community support systems.
- Strong leadership communication organizational and problem-solving skills.
- Proficiency in electronic health records and Microsoft Office applications.
Location:
This position will be split between two locations: 637 Davison Road Lockport NY and 2400 Pine Ave Niagara Falls NY. Travel between the two sites may be required.
Hours:
Monday - Friday: 8am - 5pm
Compensation:
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. This position starts at a salary of $64000-$66000 annually. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
This position entails eligibility for a sign-on bonus of up to $1500 for your commitment to work at our Horizon Certified Behavioral Health Clinics (CCBHC) located at our Pine Avenue Recovery Center and Lockport Recovery Centers for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.
Physical demands associated with this position include:
- Sitting for extended periods alternating between standing and sitting
- Mobility required with repetitive wrist hand and finger movements
- Handling light duties occasionally lifting objects up to 20 pounds
- Local travel between worksites
- Exposure to various environmental conditions including low/high temperatures outdoor elements
