We are seeking a Case Management Assistant Department Supervisor to help lead and strengthen our Case Management this role youll serve as an educator resource mentor and advocate while providing leadership and oversight to case management supervisors and administrative support staff.

What Youll Do 

  • Assist with the overall development monitoring and evaluation of the Case Management program. 
  • Provide leadership training mentorship and supervision to case management leadership and administrative support staff. 
  • Support quality assurance and regulatory compliance. 
  • Evaluate case management services including referrals intake eligibility determination program planning monitoring and assessment. 
  • Ensure required documentation assessments plans reports and financial information are completed accurately and on time. 
  • Serve as a resource to case management staff and provide coverage for supervisors when needed. 
  • Partner with county representatives service providers families guardians and other members of individuals support teams. 
  • Help ensure individuals have access to the services and resources they need to pursue their goals. 
  • Support case managers in identifying community resources and developing effective service agreements. 
  • Monitor service delivery and documentation to ensure contract and waiver requirements are met. 
  • Assist with hiring staff evaluations training and ongoing professional development. 
  • Participate in financial planning and analysis for the department. 
  • Respond to critical and emergent situations and assist with crisis response and follow-up planning. 
  • Promote positive solution-focused communication and a supportive team environment. 
  • Represent Dungarvin professionally with individuals families employees community partners funding sources and regulatory agencies. 

This is a leadership role for someone who can balance people programs compliance relationships and problem-solving while keeping the individuals we support at the center of the work. 


Qualifications :

WHAT WE LOOK FOR: 

  • Bachelors degree in a human services-related field. 
  • At least 2 years of experience working in waivered case management. 
  • At least 2 years of supervisory experience with waivered case management services. 
  • Knowledge of case management principles procedures and practices. 
  • Knowledge of local community resources. 
  • Strong written and verbal communication skills. 
  • Ability to negotiate and resolve conflict between differing parties. 
  • Ability to analyze information conduct evaluations and prepare reports and recommendations. 
  • Adaptability and the ability to move between a variety of responsibilities while maintaining efficiency and composure. 
  • Valid drivers license and proof of automobile insurance.

    Schedule & Technology: The typical schedule is MondayFriday 8:00 AM5:00 PM with flexibility occasionally needed to accommodate individuals families guardians and department needs. The position may occasionally require early morning evening or weekend availability and on-call support as directed. This position uses Microsoft Word Excel Outlook and other computer applications necessary to perform the role. 

    Driving: This is a limited-driving position. The role does not require transporting people served or driving Dungarvin-owned or leased vehicles.
