This position combines case management program coordination and direct participant support to help individuals achieve their personal goals while ensuring exceptional quality of is an excellent opportunity for someone who enjoys organization relationship building problem solving and making a meaningful impact every Youll DoCase Management & Service CoordinationCoordinate admissions and assist with the onboarding of new 3-Day Preliminary Needs Assessments for new Social Summaries and Individual Preferences Desires Needs and Strengths documents updating them with resident transitions to adult placements and other community routine visits to residential homes to monitor services and provide ongoing in community activities and events with residential Planning & DocumentationDevelop implement and maintain individualized programs for and update program data collection tools and tracking and distribute progress notes and ensure all documentation is maintained in program annual updates to individualized programs and Policy & Procedure monthly audits of program documentation and communicate findings to accurate residential data and tracking reports using Microsoft Excel and other documentation caseworker assignments and contact information & Team CollaborationSchedule and coordinate admission annual 30-day and interdisciplinary team admission meetings and lead the residential portion of interdisciplinary and distribute meeting minutes to appropriate team and distribute House Manager updates before annual required monthly Standing Committee the program during court hearings and external meetings with families attorneys social services care coordinators and other community & Quality AssuranceProvide training on annual programs and other program initiatives as quality improvement efforts through documentation reviews and compliance with residential staff and interdisciplinary teams to promote person-centered services and regulatory Youll BringRequired QualificationsHigh school diploma or GED or Bachelors degree in Human Services Social Work Psychology Rehabilitation or a related field of two years of experience providing direct or indirect support to individuals with disabilities in a healthcare rehabilitation residential or human services organizational communication and documentation to manage multiple priorities while maintaining attention to with Microsoft Office particularly to travel locally as SkillsExperience coordinating interdisciplinary of person-centered planning and individualized service working with individuals with brain injuries developmental disabilities or neurological to work independently while collaborating effectively with multidisciplinary Join NeuroRestorativeAt NeuroRestorative youll be part of a mission-driven team dedicated to empowering people to live their best lives.
