Job Title: Care Coordinator
Department: Cayuga Health Partners (CHP)
Reports To: CHP Manager Outpatient Care Coordination
Job Summary: This position provides direct care coordination for various populations in alignment with CHP network strategic goals and partnerships. The Care Coordinator collaborates with providers (inpatient and outpatient) hospitals home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery. Applicant should have strong analytical problem solving critical thinking organization and decision making skills. The Care Coordinator must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with CHP network stakeholders and partners as assigned. Must be able to prioritize coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
Job Responsibilities include:
- Promote the mission vision and values of Cayuga Health Partners.
- Actively participates in CHP trainings and activities as assigned.
- Facilitates patient access to appropriate providers and service organizations.
- Appropriately recognizes and responds to opportunities for improvement.
- Positively interacts with team members and network partners.
- Collect data as indicated to meet evaluation needs of the program.
- Prioritizes care programs as assigned.
- Routinely updates practice integration plans.
- Documents encounters with patients as assigned.
- Maintains patient confidentiality in the provision of quality care.
- Acts as representative of CHP practices as assigned.
- Appropriate communication with network partners providers patients caregivers and team.
- Participates in special projects in alignment with network goals.
- Practices always within scope - activities may be carried out within the role provided you have the knowledge skills and experience to do so lawfully safely and effectively.
- Participates in organizational and community committees as assigned.
- Regular attends team meetings - provides advanced notice if go be missed and appropriately seeks information missed.
- Is flexibility in assuming other appropriate responsibilities not noted above
Requirements:
Education - Associates Degree preferred will consider High School Diploma with 5 years of relevant experience in healthcare
Experience - Experience in acute care or related healthcare field is preferred. Demonstrating knowledge of Microsoft
Licensure - Not required.
Physical Requirements - Ability to stand sit or ambulate for long periods. Ability to mobilize freely and comfortably around units transport self to site locations and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base.

Pay Range Disclosure:
- Standard hourly rate of $27.88 - $36.00 with benefits
