Coordinator- ARCh:

Care Coordinators support families and clients by conducting regular meetings initiating contact attending home visits and team meetings and managing a clients team of support.

Care Coordinators are responsible for eligibility determination assessment of need plan development to address a childs needs and coordination of multiple services. This position is within the ARCh team which serves children and young adults up to the age of 22 who have intellectual disability autism spectrum disorder or an emotional/behavioral challenge.

ARCh - Accessing Resources for Childrenserves children up to the age of 22 with an intellectual disability autism spectrum disorder other developmental disability medical diagnoses and/or mental health diagnoses. Our main service is Care Coordination. Additional support includes service coordination with essential providers family support and specialized behavior consultation.

Responsibilities:

  • Provide a broad spectrum of services to children and young adults with an intellectual disability autism spectrum disorder and/or a mental health diagnosis
  • Per individual design assist families in identifying procuring and sustaining needed supports to ensure their ability to continue living with clients being could be accessing community resources attending school meetings supporting with transition services etc.
  • Timely and accurate documentation for service rendered needs assessments referrals etc.

Requirements:

  • Bachelors degree
  • QDDP - Qualified Developmental Disability Professional
  • On the Roster of Non-Licensed & Noncertified Psychotherapists (See Licensing Requirement Notice below)
  • 2-3 years experience in human services field with an emphasis on children with special needs
  • De-escalation skills
  • Proven team leadership ability
  • Valid drivers License and use of a personal vehicle

Licensing Requirement Notice:

Vermont regulation mandates that if you do not hold a valid clinical license then you must added to the Roster of Non licensed & Noncertified Psychotherapists. Note this process may take 3-5 days for in-state applications and up to several weeks for out-of-state applications. Instructions can be found on the website for the Vermont Office of Professional Regulation.

  • Full Time
  • Exempt
  • Annual salary starting at $44190.33
