Schedule: Sunday Thursday or Tuesday Saturday
Guaranteed Hours: 30 hours/week
Experience: Less than 1 year accepted

About the Role
We are seeking a Physical Therapist for a skilled nursing facility contract opportunity in Los Altos CA. This role involves providing therapy services to patients supporting mobility and functional recovery and helping improve overall independence and quality of life.

Responsibilities

• Evaluate patient mobility strength and functional needs

• Develop and implement individualized treatment plans

• Provide physical therapy services in a skilled nursing setting

• Support gait training balance mobility and strengthening activities

• Educate patients and care teams on therapy goals and progress

• Maintain accurate therapy documentation

• Collaborate with interdisciplinary care teams

Requirements

• Active California Physical Therapist License required

• Physical Therapist experience preferred

• Skilled nursing facility experience preferred

• Must be able to work Sunday Thursday or Tuesday Saturday schedule

• Strong communication and patient care skills required
