What Youll Do

• Deliver personalized high-quality care to older adults in homes and senior living settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions with skill and empathy.
• Collaborate across clinical disciplines (primary psych palliative neuro) for whole-person care.
• Build trusted relationships with residents caregivers and community partners.
• Maintain thorough documentation and consistent communication.

What Youll Bring

• Active unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate patient-centered approach and strong clinical judgment.
• Valid drivers license and reliable transportation.
