Our team provides essential primary care services through a network of community clinics and mobile health units caring for uninsured and Medicaid populations. Position Highlights:
- Location: Harrisonburg and Broadway VA
- Schedule: Monday Friday 9:30-6:30p. 1-2 Saturdays per month
- Working from Mobile Care RV providing direct primary care services to children and adults in underserved communities at designated locations such as Hope Distributed (Food Pantry & Clothing Ministry) and Navigation Center (Homeless Services Center).
- Partner with City Schools providing sports physicals immunizations etc.
- Shared call schedule with RN first call
- Co-manage patients with a diverse care team to include but not limited to RN Counselors Care Management and other team members to improve our patients health holistically.
- Review quality and outcome metrics monthly with the healthcare team
- Collaborate with local civic and faith-based leaders on community education programs
- Experience working in a FQHC or Community Health Center or post-masters training in Community Health Care would be of great benefit to the practice
- 1 year APP experience required
- The ability to speak Spanish is extremely helpful in this role but not required

We provide market-competitive compensation packages inclusive of base pay incentives and benefits. The base pay range for full-time employment is $100900.80 - $312000.00 annually. Additional compensation may be available for this role such as shift differentials standby/on-call pay shift premiums extra-shift incentives or bonus opportunities. Compensation within the range may vary based on qualifications experience location market conditions and business needs. Note: If an annual salary is posted it is based on a full-time colleague working 2080 hours annually.
