Job Summary

Were looking for skilled and caring Registered Nurses to join our Connecticut home care team. This role offers the autonomy of field work the support of an experienced clinical team and the chance to build meaningful patient relationships.

Full-Time opportunity

Responsibilities

• Provide skilled nursing visits and complete comprehensive assessments

• Create implement and update individualized care plans

• Deliver patient/caregiver teaching medication management and wound care

• Communicate regularly with physicians therapists and the care team

• Document all care accurately and on time

• Maintain professional boundaries and promote patient safety at all times

Qualifications

• Active Connecticut RN license (required)

• Preferred: Home health experience including OASIS

• Accepted: Hospital med-surg telemetry LTC SNF or rehab

• Strong clinical assessment skills

• Valid drivers license reliable vehicle and auto insurance

• Excellent communication and documentation skills

Schedule Options Full-Time RN

• MondayFriday

• Rotating weekend/holiday coverage
