The Denver Hospice has immediate opening for an Admissions Coordinator I.
LOCATION:Denver CO
STATUS: Full-time
SCHEDULE: Mon-Fri 8:00a-5:00p or 9:00a-6:00p rotating weekends
HOURLY PAY RANGE: $23.91 - $26.26 per hour
SUPPLEMEMENTAL PAY: Based on position schedule and/or availability:Overtime
MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile one of the highest in the industry!

THE DIFFERENCE YOULL MAKE
As a member of the Patient Access team you will often be one of the first connections patients families and healthcare partners have with our hospice and palliative care services. Your ability to listen provide clear information and respond with compassion can make a meaningful difference during an important and often difficult time.
You will coordinate referrals and support the admission process in accordance with established admission and patient access criteria helping patients and families understand available care options and connecting them with the appropriate services. You will collaborate closely with clinical business development and other team members to facilitate assessment visits address barriers to admission and create a responsive and supportive experience for patients families and referral partners.
WHO YOULL WORK WITH
Reports to the Patient Access Manager and works collaboratively with Clinical Business Development physicians healthcare providers facilities referral partners and other members of the hospice and palliative care team.

WHAT YOULL DO
- Serve as a knowledgeable and compassionate first point of contact for patients families healthcare providers referral sources and other internal and external customers seeking information about hospice and palliative care services.
- Provide clear and supportive information to patients families and healthcare partners regarding available care options including care in the home long term care and assisted living communities and hospice inpatient settings. Build effective working relationships with discharge planners referral sources and other healthcare partners.
- Work closely with Business Development Clinical and other team members to facilitate timely evaluation and access to appropriate patient care.
- Receive and accurately document complete referral information schedule assessments using the computer based appointment system identify insurance coverage and assist in coordinating the admission process.
- Collect and verify insurance coverage information and communicate relevant information to appropriate hospice team members to support timely access to care.
- Collaborate with physicians healthcare providers facilities and other members of the medical community through phone EMR systems email and other established communication methods to support responsive service and continuity throughout the referral and admission process.
- Complete required data entry accurately and efficiently. Initiate and present admission documentation to appropriate patients and families based on need payer source financial benefit and applicable legal requirements.
- Identify questions concerns or potential barriers to admission and communicate them promptly to the appropriate team member to support timely resolution and patient access.
- Represent the hospice and palliative care programs as an ambassador of the organization providing tours and information about services with professionalism compassion and sensitivity to the needs of patients and families.

- Strong computer skills including Windows based applications and the ability to effectively perform intake functions within a paperless environment and electronic medical record systems.

- The ability to manage multiple priorities and time sensitive responsibilities while maintaining accuracy organization attention to detail and follow through.
- A collaborative approach to working across disciplines and departments with flexibility professionalism and shared accountability for patient care.
- An understanding of the relationship between timely access excellent patient care the patient and family experience and organizational quality outcomes including Hospice Item Set CAHPS and other publicly reported industry and accreditation quality measures.
- Commitment to maintaining appropriate professional boundaries and protecting patient and family confidentiality in accordance with HIPAA and organizational requirements.
- Commitment to continued learning through team meetings in services continuing education professional development and annual education requirements.
WHAT YOULL NEED TO SUCCEED
- Minimum Education: High school diploma or equivalent required. Degree from an accredited college or university in healthcare business human relations or a related field preferred.
- Minimum Experience: Six (6) months of clerical and/or customer service experience required. Experience in a healthcare environment preferred. Experience supporting patients families healthcare providers referral sources or healthcare admissions is desirable.
