ACT Case Manager & FSS Worker
Job Summary
This is the entry level professional position in community based psychosocial rehabilitation services for individuals with serious and persistent mental illness. The Case Manager plans coordinates provides and monitors therapeutic rehabilitative support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible. Case Manager works independently and must demonstrate good clinical judgment decision making time management and appropriate treatment for consumers in the community.
1. Bachelors Degree in psychology social work psychosocial rehabilitation or related human services field

Associates degree in related field and two years experience working with individuals with mental illness.
2. Understanding of mental illness and desire to reduce stigma and effectively support this population.
3. Valid drivers license
4. Required to transport clients as needed
