Social Worker Hospital Discharge

Our client based in South London is seeking an experienced Social Worker to join their Hospital Discharge Team. This is an excellent opportunity to work within a fast-paced multidisciplinary environment supporting adults through safe and effective hospital discharge pathways while promoting independence wellbeing and positive outcomes.

You will work closely with health professionals carers families and community services to assess needs coordinate support and ensure individuals receive the right care at the right time following hospital admission.

KeyResponsibilities:

• Undertake Care Act 2014 assessments reviews and reassessments.
• Complete strengths-based person-centred assessments and support planning.
• Manage a caseload of adults with learning disabilities and autism.
• Coordinate care and support services to meet assessed needs and promote independence.
• Work collaboratively with multidisciplinary teams health partners carers and families.
• Advocate for service users rights choices and wellbeing.
• Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
• Identify and respond to safeguarding concerns in line with statutory procedures.
• Assess and manage risk while promoting positive outcomes and independence.
• Maintain accurate timely and legally compliant case records and reports.

Requirements

To be considered for this role you will need:

• Professional Qualification: MA BA BSc in Social Work Diploma in Social Work CQSW CSS or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
