Job Overview
A reputable healthcare facility is seeking a compassionate and experienced Registered Nurse (RN) to join its Long-Term Care/Skilled Nursing team. This role focuses on delivering high-quality patient-centered care to residents while collaborating with interdisciplinary healthcare professionals in a supportive clinical environment.
Key Responsibilities
Administer medications and treatments in accordance with physician orders and nursing standards
Conduct comprehensive nursing assessments including admissions and ongoing evaluations
Develop implement and update individualized care plans
Monitor resident conditions and document progress treatments and responses accurately
Communicate effectively with residents families physicians and care teams
Assist residents with Activities of Daily Living (ADLs) including mobility hygiene and personal care
Supervise and support nursing assistants and ancillary staff as needed
Maintain accurate clinical documentation and ensure regulatory compliance
Participate in shift reports care coordination and interdisciplinary meetings
Follow infection control safety and healthcare compliance protocols
Support staff education training and quality improvement initiatives
Apply geriatric nursing principles to support residents with complex healthcare needs
Required Qualifications
Active Registered Nurse (RN) license in the state of Arizona
Strong knowledge of long-term care and geriatric nursing practices
Experience with medication administration and patient treatment procedures
Ability to perform assessments and develop effective care plans
Knowledge of state federal and facility healthcare regulations
Excellent communication documentation and interpersonal skills
Ability to work collaboratively in a team-oriented environment
Preferred Qualifications
Minimum 1 year of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting
Experience with MDS documentation and care planning
Familiarity with EMR/EHR systems
Experience working with geriatric or veteran patient populations
