Job Summary
Position: LMSW (Licensed Master Social Worker) / MSW

Schedule
- 4x10-Hour Day Shifts (40 hours/week)
- Shift Timing: 07:30 AM 06:00 PM
- Weekend Requirement: Every other weekend
- Guaranteed Hours: 70 hours per 2 weeks
Position Summary
We are seeking an experienced LMSW / MSW with a strong background in acute care case management. The ideal candidate will bring expertise in discharge planning care coordination and the ability to work across multiple hospital units in a fast-paced healthcare environment.
Key Responsibilities
- Perform discharge planning and care coordination
- Collaborate with multidisciplinary healthcare teams
- Assess patient needs and develop individualized care plans
- Facilitate transitions of care across various settings
- Work across multiple units including Med-Surg and Emergency Department (ED)
- Document patient information using EPIC EMR
- Ensure compliance with healthcare policies and procedures
Required Qualifications
- Active Washington State LMSW license (pending accepted)
- Minimum 1 year of relevant experience
- Acute care experience required
- Experience in discharge planning and care coordination
- BLS certification required
Preferred Qualifications
- Experience in Med-Surg Emergency Department or Length of Stay (LOS) management
- Strong adaptability with the ability to float between units
