Nous sommes à la recherche dun(e) coordonnateur(trice) bilingue des soins aux patients (poste volant) afin de soutenir les programmes de soutien aux patients à travers le Canada. Dans ce rôle vous coordonnerez les services aux patients gérerez les aiguillages et veillerez à une prestation des soins fluide conformément aux niveaux de service établis.
En tant que point de contact clé entre les patients les professionnels de la santé et les équipes internes vous jouerez un rôle essentiel dans loffre dune expérience patient exceptionnelle dans un environnement de soins de santé dynamique. Ce poste exige une grande flexibilité puisque vous agirez comme ressource volante en offrant du soutien à plusieurs programmes et équipes au besoin.
Ce que vous ferez :
- Traiter les nouveaux aiguillages de patients les renouvellements et les demandes de service dans les délais établis.
- Coordonner les services aux patients y compris la prise de rendez-vous linscription et les suivis.
- Examiner et traiter avec exactitude et diligence les rapports post-visite (RPV) et les événements indésirables (EI).
- Communiquer avec les patients et les professionnels de la santé en français et en anglais.
- Maintenir une documentation précise dans les systèmes internes conformément aux normes de conformité.
- Gérer les appels entrants et sortants les courriels et les demandes de service selon les indicateurs de performance (KPI).
- Collaborer de manière transversale avec les infirmier(ère)s gestionnaires de cas pharmacies et parties prenantes des programmes.
- Soutenir la coordination des rendez-vous y compris les reprogrammations et les suivis des absences.
- Veiller à ce que les dossiers patients progressent efficacement dans le parcours de soins.
- Participer au besoin à une rotation de garde et à la couverture de quarts prolongés selon les besoins de lentreprise.
Exigences de base :
- Maîtrise du français et de langlais (oral et écrit) requise.
- Minimum de 3 ans dexpérience pertinente en services aux patients coordination des soins centre dappels ou soutien administratif.
- Expérience démontrée dans la gestion de volumes élevés de coordination de planification ou de soutien direct aux patients.
- Capacité éprouvée à gérer plusieurs priorités et à travailler de façon autonome avec une supervision minimale.
- Solides compétences informatiques (p. ex. MS Office CRM ou systèmes de planification).
Compétences / expérience souhaitées :
- Expérience dans les programmes de soutien aux patients les pharmacies spécialisées les cliniques de perfusion ou la navigation dans le système de santé.
- Connaissance de la terminologie médicale des processus de remboursement ou des flux de travail cliniques.
- Expérience dans le traitement des aiguillages de la documentation dassurance ou de la coordination de cas.
- Grand souci du détail excellentes aptitudes organisationnelles et compétences en résolution de problèmes.
- Capacité à évoluer dans un environnement dynamique axé sur des indicateurs de performance (KPI).
- Expérience de soutien à plusieurs programmes ou travail en tant que ressource volante entre différentes équipes.
Environnement de travail et autres exigences :
- Poste hybride ou à distance avec présence obligatoire au bureau une fois par mois.

About the Role:
We are seeking a Bilingual Patient Care Coordinator (Floater) to support patient support programs across this role you will coordinate patient services manage referrals and ensure seamless delivery of care within established service levels.
As a key point of contact between patients healthcare providers and internal teams you will play a vital role in delivering an exceptional patient experience in a fast-paced healthcare environment. This role requires flexibility as a floater providing coverage across multiple programs and teams as needed.
What Youll Do:
- Process new patient referrals renewals and service requests within established timelines.
- Coordinate patient services including scheduling intake and follow-ups.
- Review and process Post Visit Reports (PVRs) and Adverse Events (AEs) accurately and promptly.
- Communicate with patients and healthcare providers in both English and French.
- Maintain accurate documentation in internal systems in accordance with compliance standards.
- Manage inbound/outbound calls emails and service inquiries within KPI expectations.
- Collaborate cross-functionally with nurses case managers pharmacies and program stakeholders.
- Support appointment coordination including rescheduling and no-show follow-ups.
- Ensure patient cases move efficiently through the care pathway.
- May participate in on-call rotation and extended shift coverage based on business needs.
Basic Requirements:
- Fluency in both French and English (written and verbal) required.
- Minimum 3 years of related experience in patient services healthcare coordination call center or administrative support.
- Proven experience handling high-volume coordination scheduling or patient-facing support.
- Demonstrated ability to manage multiple priorities and work independently with minimal supervision.
- Strong computer proficiency (e.g. MS Office CRM or scheduling systems).
Preferred Skills / Experience:
- Experience in patient support programs specialty pharmacy infusion clinics or healthcare navigation.
- Knowledge of medical terminology reimbursement processes or clinical workflows.
- Experience processing referrals insurance documentation or case coordination.
- Strong attention to detail organization and problem-solving skills.
- Ability to thrive in a fast-paced KPI-driven environment.
- Experience supporting multiple programs or working as a floater across teams.
Work Environment & Other Requirements:
- Hybrid or remote role with required in-office presence once per month.
