Responsibilities:
Assist residents with ADLs (bathing dressing feeding hygiene)
Monitor and report changes in patient condition to nursing staff
Ensure patient safety comfort and overall care support
Administer medications per QMAP guidelines
Document medication administration and patient updates accurately
Support nursing staff with routine clinical tasks
Maintain cleanliness and infection control standards
Communicate effectively with healthcare team members
Requirements:
Active CNA certification required
QMAP certification required
Experience in skilled nursing facility preferred
Basic patient care and medication administration knowledge
Strong communication and teamwork skills
