Part time Registered Nurse Case Manager
Anticipated Territory : Greater Sacramento County
Schedule: Every Saturday and Sunday full days. Would consider Saturday/Sunday plus 3 week days.
What youll do:
Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:
- Conduct Start of Care (SOC) Admissions - OASIS
- Conduct follow ups evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties including reconciliation of medication wound care treatments and procedures
- Monitor clients conditions reporting changes to Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
- Accurately document observations interventions and evaluations pertaining to patient care management and services provided.

To be a success:
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Drivers License
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Previous experience with IV Medication infusions highly preferred
- Proficienciesinitiating intravenous (IV) therapy is a plus

Compensation: Per-Visit (PPV) Weekend Role
This position is compensated on a per-visit basis with rates varying by visit type.
- Visit rates range from $90 to $150 per visit depending on visit type and documentation requirements.
Expected volume: Minimum expectation of 10 visits per weekend
Estimated earnings range: up to$1500 per weekend depending on visit mix
