Chosen Village Home Healthcare is currently hiring licensed Physical Therapists (PTs) and Occupational Therapists (OTs) to provide in-home therapy services throughout the Houston Dallas/Fort Worth and Austin areas.

Travel to patients homes to provide skilled therapy services Conduct evaluations and develop individualized care plans Provide ongoing treatment and caregiver education Coordinate with patients physicians and internal case managers Complete timely clinical documentation in our EMR (HHAeXchange)
Qualifications:
Current Texas PT or OT license (required)
Minimum of 1 year clinical experience (home health preferred) CPR Certification
Reliable transportation & valid drivers license
Strong communication and organizational skills
