Your Role: Home Health LVN

As a Licensed Vocational Nurse (LVN) in Home Health you will deliver skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). Youll play a vital role in supporting patients through rehabilitation chronic condition management and senior care services.

Key Responsibilities

• Provide skilled home health nursing care based on individualized care plans
• Support senior care and rehab patients in home and community settings
• Document patient visits treatments and progress in EMR systems
• Collaborate with physicians RNs families and interdisciplinary teams
• Assist patients with chronic conditions recovery and dementia care
• Ensure compliance with California home health regulations
• Promote patient safety independence and overall well-being

Qualifications

• Active California Licensed Vocational Nurse (LVN) license in good standing
• Graduate of an accredited vocational nursing program
• Minimum 1 year of nursing experience (home health preferred)
• Experience in senior care rehab or community care preferred
• Valid drivers license reliable transportation and auto insurance
• Strong communication organization and documentation skills
• Experience with electronic medical records (EMR systems)
